The nurse supervises and directs the efforts of the Caregiver when involved in the client’s plan of care. Based on state regulations, nurses may perform administrative and/or field/clinical responsibilities.
Responsibilities
- Deliver skilled nursing care and evidence-based treatments tailored to each client's unique needs and physician-ordered plans of care
- Maintain comprehensive, accurate documentation and foster clear communication with physicians and families to ensure coordinated, person-centered care
- Expertly administer and closely monitor medications and therapeutic interventions, ensuring optimal client outcomes and safety
- Conduct comprehensive nursing assessments to establish individualized plans of care
- Maintain the highest standards of practice by consistently adhering to operational, state, and regulatory requirements
- Completes supervisory visits based on compliance and regulations to include review of plan of care, caregiver competency assessment, and completion of required documentation
- Provide compassionate, holistic nursing support that empowers clients to maintain independence and quality of life in their home environment
- Collaborate with nursing leadership and branch management to develop innovative solutions that address complex health needs and improve client outcomes
- Actively participate in staff meetings, continuing education, and professional development opportunities to stay current with best practices in homecare nursing
- Ensures compliance with physician’s verbal orders and medical reports
Qualifications
- Degree in Nursing from an accredited school
- One or more years of nursing experience in acute care, community health, or home-based settings (we welcome experienced nurses and career-changers eager to embrace homecare nursing)
- Current and unrestricted license in the state of the operation
- CPR professional certification
- Strong interpersonal and communication skills enabling you to build trusting relationships with clients, families, caregivers, physicians, and team members
- Thrive in autonomous practice with strong clinical judgment, unwavering integrity, and commitment to protecting client privacy while delivering personalized care in diverse home environments
